  3. select or list one college to receive your information
  4. determine your status (dependent or independent)
  5. if a dependent, include parents information
  6. provide personal financial information
  7. sign and submit form

Documents necessary to complete the FAFSA form

YOU, Spouse (if married) and Parents (if you are considered a dependent)

  1. social security number
  2. drivers license (if you have one)
  3. Alien registration number (if t a US citizen)
  4. Records from your untaxed income (child support, interest income, veterans non-education benefits)
  5. Bank Statements, investment, stock and bonds, real estate, business and, or farm assets
  6. Federal tax return

    • IRS 1040, 1040A, 1040EZ
    • Foreign Tax Return
    • Tax return for (Puerto Rico, Guam, American Samoa, the US Virgin Islands, Marshall Islands, The Federated States if Micronesia and, Palau)
